Chronic Disease Self-Management (CDSMP)

Looking to learn how to better manage your chronic conditions (diabetes, arthritis, heart disease, etc.), the Chronic Disease Self-Management program might be right for you. This 6-week program, available in both English and Spanish teaches participants skills and techniques for dealing with various symptoms associated with chronic conditions, such as fatigue, stress management, and frustration.


Looking to learn how to better manage your chronic pain, the Chronic Pain Self-Management program might be right for you. This 6-week program, available in both English and Spanish teaches participants how to better manage symptoms associated with their chronic pain, with an emphasis on cognitive behavioral therapy techniques for managing chronic pain.

Chronic Pain Self-Management (CPSMP)


Looking to slowly increase your physical activity or a new strategy to help manage your arthritis pain? The Arthritis Foundation’s Walk With Ease program might be right for you. This 6-week program, which can be done with a group or at your own pace, is a multi-component program that centers on walking and increasing physical activity, but also includes health education, stretching and strengthening exercises, and motivational strategies to participants.
